Service Description:

Assists individuals staying within the program, most of whom have long-term street involvement and mental health or addiction issues, to ensure housing stability. The only criteria is that the individuals have a mental health issue that is causing a barrier to success in housing

Eligibility criteria: Individuals with complex needs, experiencing chronic homelessness and living with persistent mental illness, disruptive substance use, cognitive impairment and/or concurrent disorders.
