Service Description:

A multidisciplinary team-based program providing community-based, client-centered individualized treatment, support, and rehabilitation to persons with severe and persistent mental illness. Model of service is assertive community treatment.

Application:

Referrals accepted from self, family members, and community and health professionals. Referrals are coordinated through The Access Point.

Eligibility criteria:
- Have been diagnosed with a major mental illness and have had 50 days or more of hospitalization days in the past two years.
- Substantial use of hospital and community mental health crisis or emergency services would also be taken into consideration.
