Service Description:
|
Provides an initial interdisciplinary virtual assessment is conducted and the team sets goals with the patient and tailors the program to meet their unique needs *Supports to individuals with COVID-19 who continue to experience difficulties in the following areas:- endurance
- balance
- thinking skills
- shortness of breath
- speech and swallowing
- managing difficult emotions
- managing fatigue

Eligibility / Target Population:
|
Medically stable adults with a confirmed COVID-19 diagnosis who are experiencing lingering symptoms of the virus
