Service Description: Provides case management services to individuals 16 years or older that have a mental illness (concurrent/dual diagnosis or cognitive disability included) and current involvement in the criminal justice system or who are at immediate risk of involvement with the judicial system.

Application: Referrals are accepted from all sources, including self-referrals. Referrals are through the Access Point. Please call or complete the online application form.
