Service Description: Hospice services available to clients and their loved ones and caregivers * services include:
  • resident care
  • individual palliative support
  • visiting hospice services in Northumberland County
  • caregiver support
  • support for friends and family, including grief support and respite
  • vigil support
  • information, referral and health system navigation
  • access to library resources
  • community education and training

Fees: Free
Application: Varies depending on the service * resident care requires referral from palliative care provider * call for more information
Eligibility / Target Population: Any person who has a life-threatening or life-limiting illness and requires end of life support (no age restrictions) * any person, family or friend who is experiencing or has experienced loss and is in need of support

A multi-service, volunteer-based community support organization that operates programs within Northumberland County which act to aid both clients and their caregivers. Programs operate from six local offices within the County. Call the local Northumberland Community Care office to inquire about program criteria and to pre-register for services.
