Service Description:

Community-based home visiting health care and social support services for people diagnosed with a life threatening illness and their caregivers * helps people remain independent in their own homes and
community for as long as possible * referral to other community services
Programs and services include:
- hospice palliative care services and system navigation
- grief and bereavement care
- Day Hospice program
- caregiver programs and supports
- palliative pain and symptom management consultation
- interdisciplinary and community education
- WIN Mental Health Nursing
- SELECT Nursing services for the Developmental Sector
- volunteer program
