Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors and nurse practitioner * access to nurses, dietitian, pharmacist, psychologist, social workers, phlebotomists (medical technicians who draw blood samples), foot care nurse specialist, occupational therapist, and respiratory therapist through the family health team
